If you are interested in registering as a patient, please apply directly in person at the practice.
A member of our reception team will ask you to confirm your identity with a driving license or passport, confirm your address with a utility bill, present your Health Card and complete an application form. The form can be downloaded and completed ahead of time by following the New Patient Application form links below.
When registering as a new patient at Island Medical Centre you will need to bring with you the following documentation:
If you have been in the Island for six months or more, you are entitled to a Health Card which can be obtained from Social Security. We would ask that you bring your health card to help us to process your application.
New patients joining the practice are requested to see a doctor which is a free, 10 minute registration check. If you are taking regular medication and or you have a health concern you will be advised to make an appointment at the same time.
(Secondary Practice Registration)
If you have someone staying with you and they need to see a doctor, we will be happy to see them as a temporary resident. Please enquire at reception for details.
It is very important that you notify us immediately if you change your name, address or telephone number in case we need to contact you.
